For restless legs syndrome (RLS), oral iron is usually first. IV iron is the next step when oral treatment is not enough, not tolerated, or unlikely to work fast enough.
When IV iron is considered
Clinicians may consider IV iron in moderate to severe RLS when iron markers suggest deficiency or low availability, even without anemia. Ferritin and transferrin saturation are interpreted together, not in isolation, because inflammation can make ferritin look falsely reassuring.
What gets checked first
Most plans start with a fasting iron panel: ferritin, serum iron, total iron-binding capacity, and transferrin saturation, plus CBC. Additional labs may be used when inflammation is suspected. This helps match the treatment to the biology instead of guessing.
Treatment approach
Oral iron is commonly trialed first. If symptoms persist despite an adequate trial, or if oral iron is poorly tolerated, IV formulations such as ferric carboxymaltose may be used based on clinician judgment and local protocols. Response may take weeks and sometimes requires follow-up dosing strategy.
Safety
IV iron is generally well tolerated but can cause infusion reactions, including rare serious hypersensitivity. Infusions should be done in settings prepared to manage reactions. Your clinician will choose product and monitoring plan based on your history and risk profile.
Bottom line
In RLS, iron repletion can reduce symptoms when iron availability is low. Start with proper labs, use oral iron when appropriate, and escalate to IV iron when clinically indicated.
